Patient inquiries begin with a short administrative pre-screen. The request is reviewed before the person is accepted for clinical evaluation or given access to the secure patient portal.

A narrow, defined service
An independent medical determination.
This consultation is one required part of New York’s process. The consulting physician independently examines the patient and relevant records and provides a written determination addressing the statutory criteria.
The consulting physician does not prescribe medication through this service. The attending physician remains responsible for the formal request process and any prescribing decision.
How it works

Administrative request
A short administrative pre-screen begins through our secure intake process. Do not send diagnoses, records, images, or other medical information through this public website.
Review and acceptance
Baseline reviews the administrative request for fit and capacity. A clinical record and secure portal access are created only after the request is accepted.
Secure clinical intake
After acceptance, the secure patient portal supports detailed questionnaires, consents, e-signatures, medical-record uploads, scheduling, and clinical documentation.
Evaluation and determination
The physician reviews the complete record, meets privately with the patient by secure live audiovisual telehealth, and delivers the written determination securely.
Initial eligibility
Who may request an evaluation?
- Age 18 or older and a New York State resident
- Under the care of a participating attending physician
- An incurable and irreversible illness expected to produce death within six months
- Able to make an informed decision and act voluntarily, without coercion
- Available for a private live audiovisual consultation while physically located in New York
- Willing to complete all separate statutory requirements
Every criterion is independently evaluated. Screening criteria do not guarantee confirmation.
Fees & financial access
Transparent self-pay pricing.
$250 initial clinical record review, credited toward the total.
$600 balance due before the live evaluation.
Payment covers professional review and evaluation; it does not purchase or guarantee a particular determination.
May apply when fragmented records, multiple prognostic questions, additional coordination, or accelerated review materially increases the work required.
Any adjusted fee is disclosed in advance. Accelerated availability is not guaranteed.

One complete, secure referral package.
Send the referral form, patient authorization, attending-physician information, clinical summary, and supporting records together through the approved secure referral channel.
